About Us

In Wisconsin, we’re building a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. The network is supported by a Wisconsin-based Unite Us team focused on community engagement, network health and optimization, and customer success.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Community Roots

Achieve Brown County is a network convener.

Network partners are now able to send and receive secure, electronic referrals in Arizona, Arkansas, Colorado, Illinois, Iowa, Kansas, Louisiana, Minnesota, Mississippi, Missouri, Montana, Nebraska, Nevada, North Dakota, Oklahoma, South Dakota, Texas, Wisconsin, and Wyoming through the Unite Us Platform, which ensures communities and individuals can seamlessly access much-needed resources and services across state lines.

Unite Arizona serves partners in Maricopa County, and is launching in February 2021.

Our staff was BLOWN AWAY by the quick responses to their referrals - they didn’t have to do any of the typical back and forth to get updates on the referral. In one case in particular, a partner couldn’t accept a referral for various reasons. But, instead of simply rejecting it, they sent the referral on to another organization that was able to help. This was so helpful, warm and personal!

This has been, so far, the greatest breakthrough for the entire community. Other than just sending people to services and to providers, Teenagers Plus has now gained a new momentum in the community in terms of creating linkages.

Achieve Brown County’s mission is to convene, facilitate, and support community organizations to create an equitable Brown County in which all young people have what they need to succeed in school, work, and life. Unite Us helps our community by connecting organizations and identifying solutions for community members, so they can get the help they need.

As a police social worker, I support individuals and families who are experiencing stressors from a variety of populations. The ability to have an efficient and accurate system of resources is vital to our community’s well-being. Our police department is excited to utilize the Unite Us Platform so we can find the most beneficial resource for our residents. The ability to have a seamless process to follow up, monitor progress, and maintain effective communication between providers is important to ensure individuals and families are getting the support they need.

The Greater Green Bay YMCA is dedicated to strengthening our communities in spirit, mind, and body. We are proud to have joined the Unite Us network to help share the responsibility of providing youth development, healthy living, and social responsibility to all residents of Brown County. Unite Us provides us a platform to refer young people to services that meet their needs on an individual level, interconnecting young people to experts and resources in our community.
